State law provides that any property and casualty insurance agent that is licensed by the Iowa Insurance Division can submit applications to the Iowa FAIR Plan Association. We do not appoint agents. IRS Form W-9

W-9The Iowa FAIR Plan Association does not contract with producers. No agency relationship is established and producers have no binding authority. The only requirement to be a producer for the Iowa FAIR Plan Association is to hold a property & casualty license issued by the Iowa Insurance Division.

It is required that an IRS Form W-9 Request for Taxpayer Identification Number and Certification be on file prior to the distribution of commission. Click here for a copy of the current W-9 Form.
